 1. Doctor Fills Out E-REFERRAL FORM:

Note: Cases not recommended by a physician will not be accepted

第一）医生填写电子推荐表格:  https://aspac.org.my/refer-a-patient/

备注：非医生推荐的个案，恕不受理

 

 2. Coverage Area Check:

Applications are processed only if within ASPAC covered areas

第二）凡ASPAC有覆盖的区域，申请才会受理

 3. Processing the Application:

ASPAC prepares application file. Doctor arranges appropriate home services

第三）ASPAC系统收到电子推荐表格申请资料后，

负责人会准备申请档案交给ASPAC的负责医生，医生会安排上门给予病人适当的服务

 

 4. Follow-up:

Once we have completed interviewing applicants and gathering the required information, we will accept applications that meet our criteria.

第四）一旦有关医生已经呈交电子推荐表格申请资料，我们完成对申请人的面试并收集所需信息，我们将接受符合我们标准的申请。
